Care for the Whole Person
The “golden years” can be rewarding and enjoyable as older adults find time for family and leisure activities. But sometimes isolation and longstanding physical and mental health issues can become overwhelming. You don’t have to face this alone. We’re here to help.
We offer programs that address both physical and mental healthcare needs — including our Adult Mood Program and Older Adult Transitions Services (OATS) for seniors facing life transitions, such as losing a loved one, a change in health status or a loss of independence or identity.
Personalized Primary Care for Older Adults
El Camino Health Primary Care offers comprehensive services specifically for older adults, including preventive care, annual checkups, chronic disease management, medication monitoring, and coordinating care with specialists and other caregivers.
We do more than care for you when you’re sick — we’re also here to keep you well. We find out how you want to live and what you want to accomplish and offer you clinical care and services to reach your goals. Whether it’s referring you to a specialist, offering brain health tips or connecting you with resources to help you stay independent, we help you stay in control of your health — and your life.
Our team offers a variety of services to enhance your health and wellness, including:
- Health screenings. We offer a variety of preventive health checks to help keep older adults safe, including screenings for cognitive and mental health, safe driving, fall prevention and more.
- Medicare annual wellness visits. This yearly wellness visit, covered by Medicare, creates or updates a plan to help you prevent disease or disability based on your health and risk factors. You get personalized health screenings and advice to help you feel your best.
- Advance care planning. We help you make decisions about what kind of care you want to receive in a health crisis, including assistance with advance healthcare directive and a physician order for life-sustaining treatment (POLST) forms.
- A team-based approach. We bring together professionals from a wide range of specialties to ensure you get complete care and individual attention. Your doctor works with doctors in other medical specialties, nurse practitioners, physician assistants, pharmacists, nutritionists, social workers, psychologists and therapists (physical, occupational and speech-language).
Specialized Care in the Hospital
We’re committed to providing the highest level of care for older adults, and it shows. Mountain View Hospital (El Camino Hospital) is a designated NICHE — Nurses Improving Care for Healthsystem Elders hospital. El Camino Health has achieved “Exemplar” status, the highest of four program levels, which recognizes our ability to meet the needs of the older adult patient.
NICHE is the leading and largest national geriatric nurse-driven program designed to help hospitals improve the care of older adults. People who receive care from a NICHE hospital tend to be more satisfied with their experience — and their families are more satisfied, too.
As part of the NICHE Program, El Camino Health has a nurse practitioner dedicated to improving care for older adults. Also, our clinicians have participated in training to provide high-quality, sensitive care that focuses on the needs and preferences of individuals and family members. The personalized attention we offer reflects the importance of having informed, involved patients who understand and participate in their care.
